Please submit the supplemental information for admission to the Graduate Program in Nursing listed below to the Mennonite College of Nursing Graduate Program to be considered for acceptance into a Graduate Nursing Program.

All Programs

Submit Graduate Application for Admission, two official transcripts of all prior college work, and GRE General Test scores to the Illinois State University Office of Admissions as indicated on the admissions application. The application for admission into the Master's Graduate Nursing Program is found on the Illinois State University Office of Admission website. On the application, please denote the Master's Graduate Program you are interested in.

Family Nurse Practitioner (FNP) Sequence

In addition, the following supplemental information for admission to the Graduate Program in Nursing must be sent to the Mennonite College of Nursing:

  • Intended Plan of Study form
  • Evidence of licensure as a registered nurse in Illinois;
  • Three letters of reference from persons qualified to assess the applicant's potential to succeed as a graduate student, with one letter from a nursing faculty member in the student's baccalaureate program;
  • Research Interest Form;
  • A resume; and
  • A two to three page essay outlining professional and educational goals

Nursing Systems Administration (NSA) Sequence

In addition, the following supplemental information for admission to the Graduate Program in Nursing must be sent to the Mennonite College of Nursing:

  • Intended Plan of Study form
  • Evidence of licensure as a registered nurse in Illinois;
  • Three letters of reference from persons qualified to assess the applicant's potential to succeed as a graduate student, with one letter from a nursing faculty member in the student's baccalaureate program;
  • Research Interest Form;
  • A resume; and
  • A two to three page essay outlining professional and educational goals

Gerontological Nurse Practitioner (GNP)

n addition, the following supplemental information for admission to the Graduate Program in Nursing must be sent to the Mennonite College of Nursing:

  • Evidence of licensure as a registered nurse in Illinois;
  • Three letters of reference from persons qualified to assess the applicant's potential to succeed as a graduate student, with one letter from a nursing faculty member in the student's baccalaureate program;

Clinical Nurse Leader

In addition, the following supplemental information for admission to the Graduate Program in Nursing must be sent to the Mennonite College of Nursing:

  • Evidence of licensure as a registered nurse in Illinois;
  • Three letters of reference from persons qualified to assess the applicant's potential to succeed as a graduate student, with one letter from a nursing faculty member in the student's baccalaureate program;

Nurse Educator Graduate Certificate

Applicants must meet general admission requirements as designated for the Mennonite College of Nursing Master of Science in Nursing program.In addition, the following supplemental information for admission to the Graduate Program in Nursing must be sent to the Mennonite College of Nursing:

  • Intended Plan of Study form
  • Evidence of licensure as a registered nurse in Illinois;
  • Completed Nurse Educator Graduate Certificate application form; and
  • Reference from a person qualified to assess the applicant's potential to succeed as a nurse educator.

Post Master's FNP Certificate

GRE exam scores are not required for admission to this certificate program.In addition, the following supplemental information for admission to the Graduate
Program in Nursing must be sent to the Mennonite College of Nursing:

  • Intended Plan of Study form
  • Evidence of licensure as a registered nurse in Illinois;
  • Three letters of reference from persons qualified to assess the applicant's potential to succeed as a graduate student, with one letter from a nursing faculty member in the student's baccalaureate program;
  • Research Interest Form;
  • A resume; and
  • A two to three page essay outlining professional and educational goals

Collaborative PhD Program: Focal Area Nursing in Aging

  1. Submit a completed Nursing Ph.D. admissions form, copies of any and all transcripts and copies of your GRE scores with attached documents to Mennonite College of Nursing Collaborative Doctoral Program, 205 Edwards Hall, Campus Box 5810, Normal, IL 61790-5810 by November 15, 2006.
